Access and Complete a Special Education Hearing or Mediation Survey

The Office of Administrative Hearings (OAH) offers an opportunity for feedback from individuals who participate in mediations and hearings.  As soon as the Zoom event ends, the surveys will automatically open in your browser so individuals can complete it. For hearings, as soon as the last day of hearing ends, the survey will automatically open in your browser to individuals can complete it.

If you are unable to complete the survey at the end of the Zoom event, OAH offers different ways to submit the survey at a later time. The following are the different ways to complete the surveys.

MEDIATION AND HEARING SURVEY SMARTPHONE ACCESS

You can access the online surveys for mediations and hearings using your smartphone's camera.

 

QR codes make it quick and easy to access the surveys right from your mobile device.  Below are instructions on how to use QR codes:

1. Open the camera app on your smartphone
2. Point your camera at the QR code displayed on the screen or printed material.
3. A link will appear on your device.  Tap the notification to open the survey.
4. Complete and submit the survey.

If you prefer not to use the internet, OAH can mail a copy of the survey.  Please call 916-263-0880 to request a copy.
