  • General documents:
    • EIN verification letter (if applicable)
    • Federal tax returns for the last three years (or for the period your business has been operational)
      • For a sole proprietorship, personal tax returns for up to three years.
    • DE 9C records for the last four quarters (if you have W2 employees).
    • Applicable licenses (e.g., Business License, CSLB License, Resellers Permit)
    • Award of Entitlement, eBenefits Service/Benefit Verification, or Retired/Retainer letter (DVBE only)
