The Office of Administrative Hearings (OAH) offers an opportunity for feedback from those who participated in a Department of Developmental Services mediation and/or hearing.  For those that participated, you may access our online surveys for mediations and hearings by entering the following address to your internet browser or simply click the links indicated below.
