File Late Payment Penalty Paid Report
State agencies who need assistance filing the required Late Payment Penalty Paid Report (LPPPR) should follow the procedures below. This report must show all late payment penalties paid within that fiscal year and must be reported by September 30.

Instructions to submit the LPPPR
State agencies must submit their annual Late Payment Penalties Paid Report (LPPPR) using DGS Web Forms. First-time external users will need to register for a Web Forms account using their state-issued email address. Please review the Online DGS PD 800 – Late Payment Penalties Paid Report Submission Job Aid to learn how to access and use the online LPPPR form.
Alternatives to the Web Form include:
- Emailing a digital pdf copy of the DGS PD 800 – Late Payment Penalties Paid Report to OSDSReports@dgs.ca.gov by the reporting deadline.
- Mail or in-person delivery addressed to:
