FMD Public Records Act Requests

Please read:

Public Records Act (PRA) requests for FMD documents are processed through the DGS Public Records Portal. This portal requires requesters to create an account login. FMD response emails and letters will be sent through the DGS portal, and electronic responsive documents will be saved there for retrieval.
