Effective June 1, 2026, the new web-based DSA 90 form will be used statewide across all regions.
The DSA 90 form captures the same information previously gathered by the older forms, streamlining the pre-application meeting process.
After submitting DSA 90, applicants will receive an automated email confirmation summarizing their request and next steps for submitting reference documents.
Final meeting scheduling will be coordinated directly with the appropriate DSA regional office, and any questions can be addressed to regional contacts.
