YEAR-END REPORT NO. 3, ADJUSTMENTS TO CONTROLLER’S ACCOUNTS - 7955

The Year-End Report No. 3, Adjustments to Controller’s Accounts, identifies adjustments needed to correct the central records maintained by the State Controller’s Office (SCO) as of June 30. This report does not correct the records of the SCO. Agencies/Departments must submit transaction requests through the Transaction Request Intake System (TRIS) to the SCO to correct the errors. The adjustments must also be reported on:

  • Report No. 15, Reconciliation of Agency Accounts with Transactions per State Controller.
  • Revenue adjustments will be reported on Report No. 4, Year-End Statement of Revenue.
  • Expenditure adjustments will be reported on Report No. 5, Final Reconciliation of Controller’s Accounts with Final Budget Report/Agency Records.

Agencies/Departments will report all necessary adjustments to the SCO throughout the year as soon as possible after discovering any errors. Reporting errors in a timely manner allows the SCO adequate processing time during the year and reduces the backlog of work at the end of the fiscal year.

Report No. 3 consists of the forms below and is used to report adjustments to the following accounts:

a.  Form 576A – Provided by the SCO and preprinted with commonly used legacy general ledger account numbers. Blank spaces are provided for any asset or liability account not preprinted. The form is used to report adjustments to asset and liability accounts.

b.  Form 576B – Provided by the SCO. The form is used to report adjustments to appropriation expenditure, reimbursement, transfer, and revenue accounts.

Agencies/Departments must submit an attachment (e.g., copy of the TRIS entry, SCO journal entry) to support the adjusting entries on Report No. 3. If there are no adjustments to the SCO’s accounts, Report No. 3 is not required, and the certification letter must disclose “no activity to report.” For a sample of a certification letter, see SAM Section 7951.
