New Web-Based Form DSA 90
A new web-based DSA form.
The Division of State Architect (DSA) has launched a new web-based form DSA 90 Pre-Application Meeting Request Form onto DSA's Forms and Start Construction Project by Submitting Plans for Review webpages, effective June 1, 2026.
The DSA 90 form captures the same information previously gathered by the previous forms (DSA 91-94), streamlining the pre-application meeting process. After submitting DSA 90, applicants will receive an automated email confirmation summarizing their request and next steps for submitting reference documents.
Final meeting scheduling will be coordinated directly with the appropriate DSA regional office. For questions about completing the form, please contact the regional office responsible for the project in question.
