Report Requirements

Title 2 California Code of Regulations (2 CCR), Section 8117.5 requires contract award reporting within 10 working days.  Click here to access the Contract Award Report form STD.16.

Due Dates

The Contract Award Report, STD. 16, is due within 10 days of contract award.

How to Report

The completed Contract Award Report, STD. 16, must be electronically submitted to the following e-mail address: complianceprograms@dfeh.ca.gov

Do not send paper copies of the STD. 16 to the DFEH.  Retain a copy of the completed form in the department’s procurement file.
