The Department of General Services (DGS) has eliminated the Standard Forms Fulfillment (SFF) Program effective July 1, 2017. Accordingly, the annual statewide surcharge was also eliminated effective FY 17/18.
The majority of the forms are available online as fill and save PDF’s that can also be downloaded to print as needed using your internal office printers. The forms can be accessed at the following location: https://www.dgsapps.dgs.ca.gov/osp/StatewideFormsWeb/Forms.aspx
For orders larger than your internal office printers can handle or for orders that cannot be printed on your office copiers due to special construction such as diecut, perforation, glue, etc., please contact your OSP CSR to request a custom order that will be paid for as ordered. The Office of State Publishing would like to announce the release of the Office of State Publishing’s WSF (Web StoreFront) Printed Standard Forms online portal.
The WSF allows agencies to purchase a few select pre-printed
standard forms through the user-friendly interface developed by the Office of State Publishing. The portal went live on November 1, 2017.
The following printed
standard forms are available for purchase on the WSF online portal: 117, 218Cont., 269, 350A, 404C, and 686.

In order to place an order, you will be required to register to the site. If you are already registered to the site, you will need to create second account to purchase the standard forms.
