Vehicle accident reporting information
The
Office of Risk & Insurance Management (ORIM)
administers the State Motor Vehicle Self Insurance Account (SMVIA), providing automobile liability self-insurance coverage to state employees who drive any of the State's owned or leased motor vehicles on state business.

Accident Identification
(interactive PDF) card shall be carried in the glove compartment of all State vehicles.
All vehicle accidents must be reported within 48 hours on a
STD 270
Vehicle Accident Report
(interactive PDF).
A
STD 274
State Driver Accident Review
(interactive PDF) form must be completed within five (5) days from the date of an accident. This form is required for ALL accidents regardless of who was hurt, what property was damaged, or who was responsible.
