California State Agency Temporary Telework Forms and Information
The Department of General Services (DGS) is the lead agency for the State Employees Telecommuting Program. In an effort to promote productive and safe telework practices, DGS has created telework sample forms and supporting documents that can be adapted by other State agencies in the Sacramento area that do not have formal telework programs.
These documents are intended to assist State agencies and employees who might be able to telework temporarily during the I-5 freeway repair project. For assistance with creating a formal policy for your agency's long-term use, contact the DGS Telework Program Consultant (below).
Agency supervisors are responsible for identifying and approving employees who are qualified to telework. Managers and supervisors should work with the employee to complete and retain on file the Telework Arrangement (Attachment A), the Safety Checklist/Acknowledgement (Attachment B), and the Supervisor's Checklist (Attachment C).
